                You lost your Medicare Card!

Oh No! I lost my Medicare card!
You have searched high and low, but you just cannot find your Medicare card! While you know that it just didn't grow legs and walk away, it is no longer in your wallet where it should be. You recall having to present your card to the receptionist at the doctor's office, following your visit two days ago . . . A quick telephone call reveals, however, that you did not inadvertently leave your card there. . .  Where can that darn card be? Is it lost? Worse yet, has the card been stolen? Oh no! What are you going to do now?  You need your card to obtain medical services.

There's no need to fear-- Medicare Man is here!

To replace your Medicare card, contact your local Social Security office online or call the toll-free number at 1-800-772-1213 or 1-800-325-0778 (TTY).

When to Call

You can conduct your Social Security business 24 hours a day, including weekends and holidays, by calling the toll-free number (1-800-772-1213).  Representatives are available from 7 a.m.  to 7 p.m. on business days. Representatives can handle most business needs over the telephone during normal business hours.

Lines are busiest at the beginning of the week and early in the month, so if your business can wait, it’s best to call at other times. However, if you are planning to file for benefits, you should call as soon as possible. Be sure to  have your Social Security number handy when you call.

The TTY line (1-800-325-0778) is available between 7 a.m. and 7 p.m. on business days, for anyone that is hearing impaired.  

What information do you need to request a replacement Medicare card?

  • Your last (exact) payment amount or the month and year you last received a payment if you have received benefits in the last 12 months.
  • Your name as it appears on your most recent Social Security card
  • Your Social Security Number
  • Your Date of Birth
  • Your phone number in case we need to contact you about your request
  • Your e-mail address (optional)

You may also need:

  • Your Place of Birth
  • Your Mother's Maiden Name (to help identify you)
